Who is appropriate for Assisted Living?

Diamond Willow Memory Care and Assisted Living is generally appropriate for anyone whose memory, safety, security, comfort or future well-being is compromised by remaining where they are, but who does not require the more intensive services offered in a nursing home. This includes, but is not limited to people who are feeling uncomfortable living on their own, have a poor appetite, have trouble with medications (taking, not taking, over medicating, not taking at appropriate times, etc.), have increased forgetfulness, are dependent on an outside source for day-to-day needs, lack opportunities to socialize, cannot keep up with general housekeeping or maintenance, need transportation assistance or may need help with bathing or hygiene. Diamond Willow includes services to meet all of the above needs plus assistance with dressing, bed making, regular visits by nursing staff, reminders and redirection as needed, and help getting to and from meals and activities if needed.

What types of payment do you accept?

We accept private pay, financial assistance (Medicaid/Elderly Waiver) and long-term care insurance as methods of payment at Diamond Willow. If desiring to use long-term care insurance, please be sure that your policy covers our type of facility. If you would like to learn more about whether you qualify for financial assistance, contact your county family services agency for more information.

How long does it take to move in to your facility?

Typically it takes 24-48 hours, we encourage prospective clients to call early, ask questions, set up a tour and start making a plan. There are several steps that need to happen prior to move in. Our nurses do an in person or virtual screen to assure that we can meet the needs of any potential residents. We work with your Primary Doctor to assure that all medical needs are met, as well as working with home care or hospice agencies whenever complementary care is needed.

What should I bring with me?

At Diamond Willow we encourage clients to create their space to look as much like home as possible, therefore you can bring clothing, personal care items, pictures, furniture, bed linens, and any décor or keepsakes that will make their new environment feel as familiar as able.

What is your visitor Policy?

We are currently following the guidance and recommendation of the Minnesota Department of Health and the CDC for visitations at this time. We are able to work with clients for Essential Care visits and Compassionate Care visits.

What type of care do you provide?

We provide cares for individuals with various levels of cognitive, physical, or medical needs. This means we have one all–inclusive cost that does not fluctuate based on your daily care needs.

What about Staff?

Our caring, well-trained nurses, CNAs, and PCAs are committed to providing the highest quality of service and care to our residents each and every day. Our experienced nursing staff creates every care plan to meet each individual needs. 24-hour staffing ensures comfort, well-being and peace of mind for family and friends.

What activities do we provide?

Residents may pursue the interest and hobbies of their choice. All activities are designed to encourage growth, socialization and physical activity. Activities include exercise, arts and crafts, parties, entertainment, BINGO, 1:1 reading, scripture reading and much more!
